All new students should have a physical examination, administered by a licensed medical practitioner, physician, nurse practitioner or physician's assistant. The physical exam shall have been completed within the two (2) years prior to entry into school.

Existing students are required to have a current (within two (2) years) physical on file in the health room prior to the first day of school. Physical examinations must be completed and on file every two (2) years.

Middle and High School students participating in athletics may present a copy of the DIAA Preparticipation Sports Physical Form as proof of physical. Please attach a current immunization and completed tb screening.
